You must complete this form and mail it to the appropriate County Clerk and Recorder no later than 29 days before the election. You can also use this form to update your address if you have moved.
If you are registering to vote for the first time in the State of Colorado AND you are mailing the registration application, a copy of one of the accepted types of identification must be submitted with the mail-in registration form. The accepted types of identification are listed on page 2 of the mail-in application form. Do not include original documents with the application.
In order to vote, you must be a citizen of the United States, a resident of the State of Colorado, and reside in your precinct at least 30 days prior to the election.

If you do not have identification with you when you head to the polls, you can either go get your identification and return or you can ask to vote using a Provisional Ballot.
